Community Care for Individuals with Addiction

Community Care

The goal of community care is to ensure that individuals struggling with addiction remain integrated and full-fledged members of society or successfully reintegrate into the community. To achieve this, all forms of care, psycho-social rehabilitation, and counselling are provided at the individual’s home, living environment, or place of residence.

Information Services

  • Collecting, organizing, and making accessible information related to addiction and abstinence.
  • Providing information on addiction, its possible consequences, the healing and support process, necessary steps, and expected recovery outcomes.
  • Offering internet access and computer use.
  • Providing daily and weekly newspapers.
  • Legal aid services.
  • Assisting with employment opportunities through information sharing.

Individual Case Management

  • Maintaining a continuous support relationship tailored to the individual’s needs, addiction severity, and nature, until long-term sobriety, reintegration, and the formation of natural micro-communities are achieved.
  • Skill development based on individual needs.

Problem Analysis, Problem-Solving, and Consultation

  • Helping individuals recognize and articulate their personal problems and goals.
  • Identifying motivational factors for change and encouraging transformation.
  • Problem-solving discussions.
  • Reducing the harmful consequences associated with substance use and addiction through immediate counseling and support.
  • Identifying and reaching individuals affected by addiction or at high risk.

Psycho-Social Rehabilitation

  • Assisting individuals in gaining employment.
  • Supporting the structured use of free time.
  • Facilitating the formation of recreational and self-help groups, as well as support networks.
